Individualized CARE for Older Persons with Complex Chronic Conditions at home and in nursing homes

Healthcare professionals taking care of older persons with complex chronic conditions often lack appropriate decision support. In the EU-funded I-CARE4OLD project, a multidisciplinary international team of experts will develop an innovative decision-support tool for healthcare professionals, to enable personalised care.

Our Team

I-CARE4OLD is driven by a dedicated team of experts. The characteristics of our consortium:

  • International: Europe & North-America
  • Multidisciplinary: Experts highly experienced in the fields of chronic diseases, geriatrics, long-term care, epidemiology, public health and artificial intelligence, to take into account multiple perspectives and reflecting the complexity of persons with complex chronic diseases
  • Researchers & SME's are working together towards a viable and usable solution for our target groups
  • Scientific Board to ensure validity of the research & methods
  • Stakeholder Board to ensure involvement and feedback of healthcare professionals, patients and their relatives, and relevant associations in healthcare

More than 1 in 3 people over the age of 65 live with one or more chronic conditions and require complex care. In times of increasing demand, limited resources, and staff shortages, it is crucial to provide effective and efficient care. The I-CARE4OLD project, an international and multidisciplinary research initiative funded by the European Union’s Horizon Europe program, will present an innovative decision support tool for healthcare professionals on May 14 in Amsterdam, enabling more personalized care.
